Falls Prevention Service
This service has been quality assured by Tower Hamlets Public HealthThe Falls Prevention Service in Tower Hamlets is provided through the Community Health Team. We offer individual, one to one therapy to improve your strength, balance, mobility, confidence and overall function. This will usually take place in your own home where we will address problems specific to you.
We also run a twelve week balance and strength training programmes at Royal London Hospital. These groups have a specific exercise programme tailored to you and progressed over the 12 weeks. Each session also includes discussion and advice to improve your confidence and help minimise your risk of falling. Sessions include healthy eating, bone health, foot wear and fear of falling and what to do if you have a fall.
The Community Health Team (CHT) comprises of nurses, physiotherapists, occupational therapists, care navigators, health support workers and administrative staff. You can be referred to our service by any health care professional. Our teams work closely with GPs and are based in community settings.
A therapist will come out and assess your needs and will provide you with a comprehensive home based rehabilitation programme.
Who is it for?
The Community Health Team will work with people at risk of falling or with extreme fear of falling. We tend to work with older people with decreased strength and balance but anyone over the age of 19 is eligible for referral.
The service is for any adult who:
- is a resident of Tower Hamlets
- has had one or more falls over the past 12 months
- has had a number of stumbles over the past 12 months
- has lost confidence as a result of falling or stumbling
